I need to add an internal modem (or a com port for an external modem) to an industrial PC that is running a DOS program in a pure DOS environment (not a DOS window in 98).

The PC has no avaialable ISA slots and only one Mother Board serial port. Does anyone make a NON Plug and Play PCI modem or Serial Card that will allow me to set the second serial port to Com 2 BAse Addr:02F8 IRQ4 ?

Perhaps some old PCI modems have dip switch to allow you set COM port (USR?)- not entirely sure;
The COM port you have is already in use?
If you can't find modem - you could get a PCI card that provides add'l COM ports - like
