Does anyone here have one of these (mod. # CT-5361T)? I'm trying to locate driver/diagnostic utilities for it, but am not having any luck from the manufacturer, and can't find anything online.

That is an ADSL modem - not a dial-up modem that we focus on here.

Don't believe there would be a "driver" involved with this - it would have onboard firmware.
Connecting to computer should yield a DHCP-provided IP address for your computer's ethernet connection, and a gateway. You would access the router's built-in management by using a browser with URL of the gateway IP. (you can see these addresses by opening a dos prompt and typing
ipconfig
)
