In Hyperterminal, issue ati7 to the modem and note the Product ID number, then go to USR's site and check for updated drivers.

Check in Add/Remove Programs, Windows Setup tab, highlight Communications and click the Details button, Hyperterminal should be listed there, just check it and click OK. Might check your hard drive, most often the Windows .cab files have been put in a folder on the hard drive.

Sometimes Com5 is just an alias for Com1,2,3 or 4, try each Com Port starting with Com4, enter AT, if you don't get an OK response then try Com3, then Com2, etc.

Try another way then, try the 'More Info' button in the Modem Diagnostics to see if the ati7 response is there, if so see if the Product ID is displayed.

What OS are you using?

Just download the one file that you need, save it in an empty folder on the hard drive. Then in Windows Explorer, double click the file to extract the drivers (or the file can be opened with WinZip).

Does appear to be a setup.exe included with the drivers, just be sure to check in Add/Remove Programs to see if there's a uninstaller listed to properly uninstall the modem first.
